Below are common frequently asked questions about making, changing, and cancelling an appointment.

How to make, change or cancel an appointment?

There are several ways to make an appointment at Suburban Surgery Center. An appointment request may be made online or by calling us during our business hours at (708) 453-6800 . To change or cancel your appointment, make sure to contact us during regular business hours with at least a 24-hour notice. Not showing up to an appointment or canceling last minute may result in a no-show fee.

What should I bring to the appointment?

We ask you to bring the following:

  • Form of Identification (State ID / Drivers License)
  • Insurance Card
  • Co-Payment
  • Lab results, X-Rays or any other recent information that may be required at time of appointment. Please contact us before the appointment for further clarification.

Do I have to pay for my visit at the time of my appointment?

Payment is required at time of the appointment. This includes any deductibles, co-pays, and payment for any services that are not covered by your insurance.

How much is my co-pay for my visit?

The co-pay is dependent on your insurance company. At times the co-pay is listed on your insurance card. Contact your insurance carrier to see what the co-pay would be.

I do not have insurance; can I still be seen?

Yes, we do accept self-pay patients.

How do I get my prescription refilled?

The best way to get a refill is contact the pharmacy in which your original prescription was filled. If you have issues with a prescription refill, please contact us during normal business hours of Monday – Friday 9am – 5pm.
